📋 Description

• Support care management activities and assigned member care teams • Perform member outreach by telephone and home visits • Discuss care plans/service plans, next steps, resources, questions, and concerns • Provide ongoing member education throughout care/service • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Support member and provider inquiries, requests, and concerns related to care plans/service plans • Communicate with care managers, practitioners, and other stakeholders to facilitate member services and continuity of care • Support service assessments/screenings and document member care needs • Maintain member records in accordance with state and regulatory requirements • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education about available benefits and resources • Perform other assigned duties and comply with company policies and standards

🎯 Requirements

• High School diploma or GED required • 1–2 years of related experience required • For Arkansas Total Care: Bachelor’s degree in social science/health-related field, or high school diploma with at least 1 year of experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients • Must have a driver’s license • Child and adult maltreatment check required before hire and recurring • Drug screen required at hire and recurring • Must reside in Arkansas or a border city • Must be able to support members throughout Lincoln, Cleveland, Drew, and Bradley counties in Arkansas • 30% travel required • Strong experience working with the Developmental Disabilities population preferred • Ability to coordinate care and make appropriate referrals • Ability to document member records and care needs in compliance with state and regulatory requirements
