📋 Description

• Develop, assess, and facilitate complex care management activities for members with primarily mental and behavioral health needs • Create personalized care plans and provide education for members and families related to mental health and substance use disorder • Evaluate member needs by phone or in-home visits and recommend or facilitate care and service plans • Conduct telephonic, digital, home, and other site-visit outreach to assess needs and collaborate with resources • Develop ongoing care plans for high-acuity members and identify providers, specialists, and community resources • Coordinate among members, families or caregivers, community resources, and care provider teams • Monitor care plans, member status, treatment side effects, complications, and clinical symptoms • Facilitate care coordination with providers and specialists to ensure timely access to care • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care and quality delivery cost-effectively • Perform other duties as assigned and comply with policies and standards

🎯 Requirements

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master's Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN based on state contract requirements • Behavioral health experience required for RN candidates • Ability to develop and manage complex behavioral health care plans • Ability to evaluate member needs through phone and in-home visits • Ability to coordinate with providers, specialists, families, caregivers, and community resources • Ability to document care management activities in compliance with state, federal, and third-party payer regulations • Monday–Friday availability, 8:00 a.m.–5:00 p.m. • LCSW, LMSW, LMFT, LMHC, or LPC license strongly preferred
