📋 Description

• Develop, assess, and coordinate complex discharge plans for high-risk members based on needs assessments • Facilitate smooth transitions from acute care to the next level of care • Educate members and families/caregivers on discharge plans • Coordinate with providers, community agencies, and utilization review staff to develop safe, effective, clinically appropriate transition plans • Manage discharge plans across levels of care and facilities • Collect, document, and maintain discharge plans in member medical records and health management systems • Provide education on discharge plans and utilization processes • Provide leadership feedback on discharge planning process improvements • Coordinate and assist with prior authorizations related to discharge planning • Assist the concurrent review team as needed • Perform other duties as assigned • Comply with all policies and standards

🎯 Requirements

• Graduate from an Accredited School of Nursing or Bachelor’s degree in Nursing • 2–4 years of related experience • 2+ years of acute care experience required • Active Compact RN License • RN - Registered Nurse - State Licensure and/or Compact State Licensure • Clinical knowledge of discharge planning competencies preferred • Knowledge of Medicare and Medicaid regulations preferred • Knowledge of utilization management processes preferred
