📋 Description

• Develops, assesses, and coordinates holistic care management activities for long-term care members • Develops or assists with developing personalized service care plans/service plans • Educates members and families/caregivers on available services, benefits, procedures, healthcare provider instructions, service options, referrals, and healthcare benefits • Evaluates member needs and available resources, and recommends or facilitates plans for best outcomes • Develops ongoing long-term care plans/service plans • Identifies providers, specialists, and community resources needed for long-term care • Coordinates between members/families/caregivers and care provider teams to ensure services are accessible • Provides resource support for employment, housing, participant direction, independent living, justice, and foster care needs • Monitors care plans, member status, and outcomes, and recommends updates based on member needs • Interacts with long-term care healthcare providers and partners • Collects, documents, and maintains member information and care management activities for regulatory compliance • Performs home and/or other site visits to assess member needs and collaborate with providers and partners • Provides feedback to leadership on opportunities to improve quality of care and cost-effective service delivery • Performs other duties as assigned

🎯 Requirements

• Must reside in Miami Dade County, FL • Bachelor's degree and 2–4 years of related experience; or Bachelor's degree in health, human, social work or education services with one or more years of qualifying experience; or high school degree or equivalent and three years of qualifying experience with case management of the aged, including management of behavioral health conditions, persons with physical or developmental disabilities, or HIV/AIDS population • Experience interviewing and assessing member needs • Knowledge and experience regarding caseload management and casework practices • Knowledge regarding determining eligibility for DHSS programs • Knowledge regarding Federal and State law as it applies to DHSS programs • Ability to effectively solve problems and locate community resources • Ability to collaborate with caregivers, involved State agency representatives and providers • Good interpersonal skills • Ability to practice Cultural Competency with awareness and respect for diversity • Knowledge of the needs and service delivery system for all populations in the case manager’s caseload • Field-based work • Technological savvy • Excellent customer service skills • Hospice experience • Department of Children and Family experience • Excellent written and oral communication skills • Case management • Bilingual (Spanish) • Organizational skills • Complies with all policies and standards
