• Work with members and providers to close behavioral health care gaps and remove barriers to care • Improve member and provider experience through targeted outreach • Collaborate with members and providers to improve behavioral health HEDIS measures • Support behavioral health quality-improvement interventions • Conduct telephonic outreach, education, and health coaching • Schedule doctor and follow-up appointments • Arrange transportation and connect members with community-based resources and affinity programs • Act as liaison and member advocate between members/families, physicians, and facilities/agencies • Directly close FUH, FUM, and FUA HEDIS care gaps • Assist members in finding and connecting with behavioral health providers • Document member contacts and actions taken • Interact with customer service and other departments to resolve member issues • Refer members to case or disease management as appropriate • Provide coaching and training to Quality Behavioral Health Advocates • Act as subject matter expert for departmental best practices, reports, and data extracts • Lead preparation and submission of projects, reports, and assignments • Assist with new and expanding healthcare QI/HEDIS initiatives • Represent the supervisor in meetings and special assignments • Establish process improvements and assist with implementing departmental change • Conduct audits and chart reviews of fellow advocates • Participate in community, health department, collaborative, and organizational quality-improvement meetings • Complete special assignments and other duties as assigned
• Applicants must reside in the state of FL • Active FL mental health/behavioral health licensure or RN with behavioral health experience • Bachelor's Degree in Healthcare, Public Health, Nursing, Psychology, Social Work, Health Administration, or related health field, or equivalent work experience • A total of 4 years of experience required for the position • 4+ years of experience in direct patient care, social work, quality improvement, or health coaching, preferably in a managed care environment • Required upon hire: one of LCSW, LMSW, Licensed Psychologist, RN-BC, PMHNP-BC, MFT, LMFT, LMHP, LPC, LMHC, PA-C with behavioral health experience, MD psychiatrist, LISW, LICSW, LCPC, LIMHP, LPCC, or LPCMH • Ability to work Monday–Friday, 8am–5pm Eastern Time • Ability to maintain confidentiality of business and protected health information • Compliance with all policies and standards
