📋 Description

• Assess, plan, and implement complex care management activities for the NH Health Families Medicaid population • Develop personalized care plans/service plans and address member needs • Educate members and families/caregivers on services, benefits, disease processes, care options, referrals, and healthcare instructions • Collaborate with providers, specialists, community resources, caregivers, and care teams to coordinate person-centered care • Monitor member status, changes in condition, progress, and care-plan goals; revise plans as needed • Identify barriers and appropriate care-management interventions for escalated cases • Review member data, referrals, and intake assessments to improve quality, performance, and care plans • Perform telephonic, digital, home, and/or other site outreach • Collect, document, and maintain member information and care-management activities for regulatory compliance • Partner with leadership to improve cost-effective care and quality delivery • May precept clinical new hires, provide coaching and shadowing, support onboarding, and assess competency/readiness • Perform other duties as assigned and comply with policies and standards

🎯 Requirements

• Degree from an accredited school of nursing or a Bachelor's degree in Nursing • 4–6 years of related experience • RN state licensure and/or compact state licensure required • NH RN licensure required • Experience in both Physical Health and Behavioral Health highly preferred • Knowledge of care management, Medicaid services, healthcare benefits, referrals, and person-centered care • Ability to develop, monitor, and revise care plans/service plans • Ability to collaborate with members, caregivers, healthcare providers, specialists, community resources, leadership, and cross-functional teams • Ability to document care-management activities and comply with state, federal, and third-party payer regulations
