📋 Description

• Assess, plan, and implement complex care management activities • Develop and continuously assess personalized care plans/service plans • Collaborate with providers, specialists, community resources, members, families, caregivers, and care teams • Coordinate and manage member care and services • Monitor member status, condition changes, progress, and care-plan goals • Identify problems, barriers, and interventions for escalated cases • Review member data, referrals, and intake assessments • Perform telephonic, digital, home, and/or site outreach • Collect, document, and maintain member and care-management information for regulatory compliance • Educate members and caregivers on disease processes, care gaps, treatment instructions, care options, referrals, and benefits • Partner with leadership to improve care quality and cost effectiveness • Precept, coach, and support clinical new hires during onboarding • Complete competency checkpoints and monitor preceptee readiness • Collaborate with People Leaders and cross-functional teams • Perform other assigned duties and comply with policies and standards

🎯 Requirements

• Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing • 4–6 years of related experience • RN - Registered Nurse - State Licensure and/or Compact State Licensure required • NY RN State Licensure required • Ability to work Monday–Friday, 8am–5pm EST
