• Assess, plan, and implement complex care management activities • Develop personalized care plans/service plans for members • Address member issues and educate members and families/caregivers on services and benefit options • Assess ongoing care plans and collaborate with providers and community resources • Coordinate members, caregivers, and care provider teams to ensure appropriate person-centered care • Monitor member status, changes in condition, progress, and care-plan goals • Identify barriers and interventions for escalated cases • Review member data, referrals, and intake assessments to improve quality and operating performance • Perform telephonic, digital, home, and/or other site outreach • Collaborate with healthcare providers to facilitate services and treatments • Collect, document, and maintain member and care-management information for regulatory compliance • Provide education on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits • Partner with leadership to improve care and quality delivery cost-effectively • May precept clinical new hires, provide coaching, facilitate growth, and guide onboarding • Support clinical new hires/preceptees in navigating a Managed Care Organization • Complete competency checkpoints and monitor readiness for Service Coordination success • Collaborate with People Leaders and cross-functional teams to measure and monitor readiness • Perform other duties as assigned and comply with company policies and standards
• Degree from an accredited school of nursing or a Bachelor's degree in Nursing • 4–6 years of related experience • RN state licensure and/or compact state licensure required • WA State RN licensure and/or compact state licensure required • Ability to work Monday–Friday, 8:00 am–5:00 pm Pacific Standard Time • Compliance with state, federal, and third-party payer regulations • Knowledge of care management, personalized care plans, member assessment, healthcare benefits, referrals, and provider coordination
