• Manage overall care management and quality of care for participants • Review field staff assessments and discipline-specific documentation for quality and address deficiencies • Participate in care planning meetings and represent the dietitian discipline • Monitor field staff documentation and interventions; conduct coaching, competency, and training sessions • Coordinate a continuum of care with discipline-specific staff and the interdisciplinary team (IDT) • Arrange, coordinate, and authorize appropriate member-specific services, including ADLs/IADLs assistance, housing, meals, and transportation • Implement care management activities and interventions addressing participant goals • Coordinate care across sites and during transitions between care settings • Document services according to CLHC standards and federal/state regulations • Coordinate long-term care services in nursing homes, rehabilitation facilities, and other settings • Collaborate with primary care providers, specialty physicians, specialty services, members, and IDT • Provide or arrange skilled services, service authorization, periodic reassessment, and evaluation • Monitor care management activities, member responses, effectiveness of the care plan, and service utilization • Modify care plans based on changes in member health and needs • Maintain current knowledge of health issues, procedures, evidence-based guidelines, medications, and practice standards • Recommend improvements to services, programs, policies, and procedures • Follow disciplinary policies and engage Human Resources as needed • Act within scope of authority and meet PACE competencies before working independently • Serve as an acting member of the IDT • Perform other duties as assigned
• Baccalaureate degree in nutrition or related field • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience preferred • Customer Service experience required • Managed long-term care insurance experience beneficial • Be legally authorized to practice in the State in which the healthcare professional will perform the function • Minimum of one (1) year of experience working with a frail or elderly population, or receive appropriate PACE organization training upon hiring if under one year • Current active and unrestricted license and registration in specialty in New York State, required • Be medically cleared for communicable diseases and have all immunizations up-to-date before direct participant contact • Ability to meet required physical requirements, including standing, sitting, lifting/pushing/pulling up to 50 pounds, bending/squatting, walking/climbing, fine motor skills, visual accuracy, hearing and speaking clearly, and cognitive decision-making • Bilingual preferred
