📋 Description

• Responsible for overall care management and quality of care for participants • Review field staff assessments and discipline-specific documentation for quality; address deficiencies • Participate in care planning meetings and represent the nursing discipline • Monitor field staff documentation and conduct coaching, competency, and training sessions • Coordinate a holistic, continuous care plan with discipline-specific staff and the interdisciplinary team (IDT) • Arrange, coordinate, and authorize services including ADLs/IADLs assistance, housing, meals, transportation, skilled services, and long-term care • Collaborate with physicians, specialty services, facilities, discharge planners, home care coordinators, members, and IDT staff • Provide care management across sites and coordinate transitions between care settings • Document services according to CLHC standards and federal/state regulations • Monitor interventions, service utilization, member responses, outcomes, and modify plans of care as needed • Maintain current knowledge of health-related issues, procedures, evidence-based guidelines, medications, and practice standards • Recommend improvements to services, programs, policies, and procedures • Follow disciplinary policies and engage Human Resources as needed • Act within scope of practice and meet PACE competencies before working independently • Serve as a member of the IDT • Perform other duties as assigned

🎯 Requirements

• Graduated from a Nursing education program acceptable to New York State Education Department (NYSED) • Current active and unrestricted license and registration in New York State required • Minimum of two (2) years of Care Planning Experience • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred • Customer Service experience required • Minimum of one (1) year of experience working with a frail or elderly population, or appropriate PACE organization training upon hiring if under one year • Legally authorized to practice in the state where the healthcare professional performs the function • Medically cleared for communicable diseases and immunizations up-to-date before direct participant contact • Bilingual preferred • Ability to meet the stated physical, mobility, lifting, sensory, cognitive, and travel requirements • Bachelor's degree preferred • Managed long-term care insurance experience beneficial • Supervisory experience preferred
