📋 Description

• Manage overall care management and quality of care for participants • Review field staff assessments and discipline-specific documentation for quality and address deficiencies • Participate in care planning meetings and act as a member of the IDT • Monitor field staff documentation and conduct coaching, competency, and training sessions • Coordinate a continuum of care aligned with members’ healthcare needs and goals • Arrange, coordinate, and authorize appropriate member-specific services, including ADL/IADL assistance, housing, meals, and transportation • Implement care management activities and interventions to achieve participant goals • Coordinate transitions between care settings with team members, facilities, discharge planners, and home care coordinators • Document services according to CLHC standards and federal/state regulations • Coordinate long-term care services in nursing homes, rehabilitation facilities, and other settings • Collaborate with PCPs, specialty physicians, specialty services, and the IDT regarding participant condition changes • Provide or arrange skilled services, service authorization, periodic reassessment, and service evaluation • Monitor care management activities, services, member responses, plan effectiveness, and service utilization • Modify plans of care based on changes in member health and goals • Maintain current knowledge of health issues, procedures, evidence-based clinical guidelines, medications, and practice standards • Recommend improvements to services, programs, policies, and procedures • Follow disciplinary policies and engage Human Resources as needed • Act only within the scope of authority to practice • Complete required position competencies before working independently • Work 8:00 AM–5:00 PM, 40 hours weekly

🎯 Requirements

• Graduated from a Nursing education program acceptable to New York State Education Department (NYSED) • Bachelor's degree preferred • Minimum of two (2) years of Care Planning Experience • Minimum of two (2) years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred • Customer Service experience required • Managed long-term care insurance experience beneficial • Minimum of one (1) year of experience working with a frail or elderly population, or receive appropriate PACE organization training upon hiring if having less than one year while meeting other requirements • Supervisory experience preferred • Current active and unrestricted license and registration in New York State required • Be legally authorized to practice in the state where the healthcare professional will perform the function • Be medically cleared for communicable diseases and have all immunizations up-to-date before direct participant contact • Bilingual preferred • Meet physical, cognitive, visual, auditory, mobility, lifting, and fine motor requirements of the position
