📋 Description

• Manage overall care management and quality of care for participants using discipline-specific knowledge. • Review field-staff assessments and documentation for quality, address deficiencies, and monitor interventions. • Participate in care-planning meetings and act as a member of the interdisciplinary team (IDT). • Coordinate holistic care plans and communicate regularly with discipline-specific field staff. • Arrange, coordinate, and authorize services addressing member needs, including ADLs/IADLs, housing, meals, transportation, skilled services, and long-term care. • Collaborate with physicians, IDT members, facilities, discharge planners, and home care coordinators across care settings. • Implement, monitor, evaluate, and modify care-management interventions and plans based on member outcomes and changing health needs. • Document services according to CLHC standards and federal/state regulations. • Conduct coaching, competency, and training sessions with field staff. • Recommend improvements to services, programs, policies, and procedures. • Follow disciplinary policies and engage Human Resources as needed. • Act within the scope of practice and complete required competencies before working independently. • Perform other duties as assigned.

🎯 Requirements

• Graduated from an Occupational Therapy program acceptable to New York State Education Department (NYSED.) • Minimum of 2 years of administrative experience in a management capacity in a certified home health agency (CHHA), Rehab, long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred. • Customer Service experience required. • Managed long-term care insurance experience beneficial. • Minimum of one (1) year of experience working with a frail or elderly population, or appropriate PACE-organization training upon hiring if less than one year and all other requirements are met. • Supervisory experience preferred. • Be legally authorized to practice in the State in which the healthcare professional will perform the function. • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact. • Ability to meet stated physical requirements, including standing, sitting, lifting/pushing/pulling up to 50 pounds, bending/squatting, walking/climbing, travel between sites, fine motor skills, visual accuracy, hearing/speaking, and cognitive decision-making abilities.
