📋 Description

• Act as mental health support for referred patients and those interested in mental health services • Message patients about behavioral health questions through the Diana Health platform • Conduct in-office and telehealth mental health visits via warm hand-off from providers and/or patient request • Provide ongoing mental health treatment and refer out as needed • Complete timely clinical documentation in the EHR system • Collaborate with CNM/OB-Gyn on plans of care, escalation areas, and medication needs • Collaborate with the Health Coach regarding patient wellness • Collaborate with the Care Navigator to provide additional patient support • Provide staff training on behavioral health and workflows • Lead behavioral-health expert talks and support groups • Connect patients and providers to external referrals and resources

🎯 Requirements

• Licensed Clinical Social Worker (LCSW) or Marriage Family Therapist (MFT) with current Tennessee licensure • 2+ years of experience providing mental health support to a similar client panel • Excellent communication, interpersonal, and organizational skills • Strong computer skills and familiarity with EMRs • Specialize in postpartum or trauma or offer EMDR or IFS therapy, etc. • Use a therapy modality other than CBT or DBT • Must have a Tennessee license or be able to acquire one upon offer
