📋 Description

• Provide skilled pediatric physical therapy to children from birth through age 21 in home and community settings • Evaluate mobility, strength, balance, coordination, gross motor development, positioning, and functional movement needs • Develop individualized plans of care supporting safety, independence, and functional progress • Partner with families and caregivers to promote carryover between visits • Collaborate with occupational therapy, speech therapy, nursing, and care coordination teams • Complete timely documentation using electronic charting tools • Build a caseload around preferred schedule and service areas • Provide one-on-one care in children’s natural environments

🎯 Requirements

• Doctor of Physical Therapy or related PT degree • Active or eligible Colorado Physical Therapist license • Current in-person CPR/BLS certification • Valid driver’s license • Auto insurance • Reliable transportation • Pediatric experience preferred; new graduates encouraged to apply
