• Provide physician services to the Patient Access Department to facilitate timely access to hospice care • Provide physician direction and education to admissions personnel • Assist with obtaining additional information to establish patient eligibility • Complete regulatory hospice physician documentation for each admission • Collaborate with the Director of Patient Access Services and Patient Access/Admissions Department • Review, confirm, and evaluate patient information from medical records, referring or primary physicians, and admissions nurses • Determine initial and ongoing hospice eligibility using prognostic indicators, Local Coverage Determinations, coexisting conditions, and disease progression • Provide verbal certification of hospice eligibility • Complete written certification and clinical narratives for eligible admitted patients • Order medications promptly to promote comfort for newly admitted patients • Evaluate the admissions process and recommend efficiency and accuracy improvements • Educate Patient Access/Admissions and Professional Relations Departments on eligibility information and regulatory changes • Determine related/unrelated medications, treatments, and hospitalizations and document them in the EMR • Review hospital patients for appropriate levels of care and participate in hospital interdisciplinary groups • Participate in quality improvement, research, educational, and professional/community activities • Perform other duties as assigned
• MD or DO from an accredited medical school • Licensed to practice medicine in the State of Florida, with willingness to obtain additional states as required • Current DEA number for Florida and additional requested states • State Controlled Substances Registration in Maryland and District of Columbia required • Current Medicare and Medicaid Numbers • Active BLS certification completed by either American Heart Association or the Red Cross, with in-person training • Minimum of five (5) years’ experience or Fellowship • Eligible for malpractice coverage under Chapters’ policy terms • Knowledge and experience in palliative medical practice and symptom management • Basic computer skills including email and word processing (Microsoft Outlook and Word or equivalent) • Must be able to physically access any home in the organization’s service area • Ability to manage emotional stress when working with terminally ill patients and their families • Consent to drug and/or alcohol testing after a conditional offer and ongoing compliance with the Drug-Free Workplace Policy • Florida Care Provider Background Screening Clearinghouse eligibility for services performed for Florida affiliates • Hospice and Palliative Medicine certification, ABMS board certification, and recognized ABMS field board eligibility/certification are preferred
