📋 Description

• Entry-level remote position supporting vulnerable populations by managing incoming calls and coordinating referrals to appropriate levels of care. • Responsibilities include assessing eligibility for placement, communicating with referral partners and guardians, and providing trauma-informed support, including de-escalation when needed. • Requires multitasking across systems, navigating EHRs, and collaborating with internal and external teams to ensure timely, accurate outcomes. • Key Responsibilities: Provide respectful, inclusive interactions with clients, families, and community partners. • Manage intake calls, walk-ins, and referrals; complete timely and accurate assessments. • Coordinate placements or referrals to appropriate levels of care. • Maintain strict confidentiality and accurate documentation in data systems. • Communicate effectively with health plans, referral partners, and internal teams. • De-escalate crisis situations using trauma-informed approaches. • Collaborate with multidisciplinary teams to support care coordination. • Navigate multiple systems (EHR, Teams, etc.) to ensure efficient workflow. • Attend trainings, meetings, and stay current on policies and procedures. • Demonstrate professionalism, attention to detail, and commitment to diversity, equity, and inclusion.

🎯 Requirements

• Education: Bachelor’s degree in a relevant field (i.e. Social Work, Education, Sociology, Psychology, Counseling, Human Relations, or Criminal Justice) preferred. • A high school diploma or equivalent with a minimum of two years of relevant experience (e.g., working with vulnerable populations, intake coordination, or crisis management may be considered. • Licensure/Certification: Valid drivers license and auto insurance. At least 21 years of age. • Experience: One or more years of related experience. Supervisory experience preferred. • Preferred candidates will have at least two years working with vulnerable or at-risk populations, along with strong technical skills in Microsoft 365, cloud-based platforms, and Electronic Health Records systems. • Candidates should be comfortable managing multiple digital tools simultaneously while demonstrating excellent multitasking, organization, and time management abilities. • Strong verbal and written communication skills are essential, as is the ability to work independently, problem-solve, and collaborate effectively in a remote environment. • Experience with crisis intervention, de-escalation techniques, and trauma-informed care is highly valued, along with a high level of attention to detail when handling intakes and patient data.
