📋 Description

• Engage Medicare members through phone, text messaging, and other approved digital channels • Promote positive health outcomes • Conduct comprehensive assessments and use motivational interviewing • Provide education and behavior-change expertise • Develop individualized intervention plans • Help members address complex barriers to care • Improve medication adherence and completion of preventive health activities • Support management of chronic conditions • Apply advanced coaching techniques, critical thinking, clinical knowledge, and independent judgment • Partner with members, providers, pharmacies, caregivers, and internal teams • Support personalized care plans, member engagement, self-management skills, and sustained health improvement • Meet quality, compliance, documentation, and productivity expectations

🎯 Requirements

• Three or more years of healthcare, health coaching, care management, case management, wellness coaching, or related field • Experience supporting individuals with chronic conditions, preventive care, medication adherence, or health education • Experience using Microsoft Office, web-based applications, and multiple business systems • Must have a HIPAA compliant workspace • Ability to hard-wire connect to the internet • Associate's degree or an equivalent combination of education and relevant experience required
