📋 Description

• Provide In-Home Health Evaluations in members’ homes • Build trusted relationships and help improve health outcomes • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered, including diabetic eye exams, spirometry, and bone density screening • Use an iPad and intuitive clinical workflow for each evaluation • Conduct virtual visits as needed • Provide Diagnostic & Preventive Services products • Travel locally and across licensed states, with possible overnight hotel stays, car rentals, and flights • Visit people nationwide and connect them to appropriate care

🎯 Requirements

• Nurse Practitioner or Physician Assistant • Active, unrestricted license(s) in coverage area(s) • Board certification (required) • Able to obtain and maintain multi-state licenses in coverage area, as required • Able to cover all counties within state and flex to other areas of business need • Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants • Willing and able to travel locally and within licensed states, including up to 100% travel • Able to conduct virtual visits as needed
