📋 Description

• Applies advanced clinical judgement to facilitate appropriate member physical health and behavioral healthcare • Implements strategy to assess and plan care • Directs provider collaboration • Coordinates psychosocial wraparound services • Instructs the application of available resources to produce optimal, cost-effective outcomes

🎯 Requirements

• Case Management Experience • Strong Customer Service Orientation and Problem Solving • Excellent Communication/Telephonic Skills • Excellent Motivational Interviewing Skills/ability to build rapport and trust telephonically • Excellence in Documentation and respecting compliance/regulatory standards • Highly Organized, remain self-driven without direct supervision • Manage Time Effectively • Achieve performance metrics • Technology Proficiency • Variable work schedules with ability to work 2 days weekly until 9pm local time • Master's plus BH Clinical License in good standing with no restrictions required (LPC, LMFT, LCSW) • Master's Degree • Two years Care Management experience preferred • CCM Certification preferred • Previous Experience in Managed Care Organization preferred
