📋 Description

• Facilitate appropriate healthcare outcomes for members • Provide care coordination, support and education for members • Use care management tools and resources • Travel throughout the assigned Lake County, Illinois field area up to 75% of the time

🎯 Requirements

• 2 or more years of experience in behavioral health, social services or human services field • 2 or more years of case management experience • Reliable transportation • Willingness and ability to travel up to 75% of the time • Valid Illinois Driver’s license • Bachelor’s degree or non-licensed master level clinician required • Degree in behavioral health or human services required, such as psychology, social work, marriage and family therapy, or counseling • Preferred: discharge planning experience • Preferred: Managed Care Organization experience • Preferred: Microsoft Office experience
