📋 Description

• Use collaborative assessment, planning, facilitation, care coordination, evaluation, and advocacy to address individuals’ and families’ comprehensive health needs • Communicate with individuals, families, and available resources to promote quality and cost-effective outcomes • Perform in-person case management activities within a designated geographic area when directed by leadership or business needs • Conduct case management, discharge planning, and/or home health care coordination • Work Monday–Friday, 8:00 AM–5:00 PM Eastern

🎯 Requirements

• Compact/New York or Massachusetts RN license required • Minimum 3–5 years clinical practical experience preferred, including diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac experience with Medicare members • Minimum 2–3 years of case management, discharge planning and/or home health care coordination experience • Ability to occasionally travel within a designated geographic area for in-person case management activities • Bilingual preferred • Excellent analytical and problem-solving skills • Effective communication, organizational, and interpersonal skills • Ability to work independently, including from home • Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications • Efficient computer skills, including navigating multiple systems and keyboarding • Willingness and ability to obtain multistate RN licenses if needed • Active Registered Nurse state license in good standing within the region where job duties are performed required • Certified Case Manager preferred • Additional national professional certification such as CRC, CDMS, CRRN, COHN, or CCM preferred but not required
