📋 Description

• Conduct comprehensive behavioral and physical health assessments using clinical tools and data review • Develop and coordinate individualized care plans aligned with member benefits and available resources • Collaborate with providers and internal teams to ensure seamless, cost-effective care • Provide crisis intervention and follow-up support for members experiencing behavioral health or medical crises • Use Motivational Interviewing and advanced clinical judgment to reduce risk factors and improve outcomes • Coordinate psychosocial wraparound services and community resources • Support members with complex behavioral and physical health needs from home

🎯 Requirements

• Active, unrestricted independent clinical license in your state (LCSW, LSW, LISW, LCPC, LPC, LP, LMFT, or LMHC) • 3+ years of post-master’s direct clinical experience in a hospital, outpatient, ambulatory, or similar setting • 3+ years of experience with mental health and substance use disorders • Comfort providing care telephonically while documenting in real time • Strong Motivational Interviewing skills • Strong technology skills • Master’s degree in Social Work or Counseling • Crisis intervention experience preferred • Managed care or utilization review background preferred • Case management and discharge planning experience preferred
