📋 Description

• Provide Part Time In-Home Health Evaluations in members’ homes • Engage with people and help bridge gaps in care • Conduct approximately 45-minute visits • Review medications and medical history • Perform physical evaluations • Provide additional diagnostic tests when ordered by the health plan, such as diabetic eye exams and spirometry • Use an iPad and intuitive clinical workflow for each evaluation • Build trusted relationships and improve health outcomes • Visit health plan members nationwide and connect them to appropriate care • Travel locally and within licensed states as needed • Conduct virtual visits as needed • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Work primarily in the private residences of health plan members

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Board certification required • Multi-state licenses preferred • Nurse Practitioner or Physician Assistant qualification • Ability to travel locally and within licensed states as needed • Ability to conduct visits safely in private residences that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations • Ability to perform medication and medical history reviews, physical evaluations, and diagnostic and preventive services • Anticipated weekly hours: 29.5
