📋 Description

• Provide approximately 45-minute in-home health evaluations • Engage with members one-on-one and answer health questions • Review medications and medical history • Perform physical evaluations • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry • Build trusted relationships and help improve health outcomes • Visit people nationwide and connect them to appropriate care • Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening • Conduct virtual visits as needed • Travel locally and within licensed states as needed • Work primarily in health plan members’ private residences

🎯 Requirements

• Active, unrestricted license(s) in coverage area(s) • Multi-state licenses preferred • MD or DO qualification • Ability to travel locally and within licensed states, as needed • Ability to conduct virtual visits as needed • Ability to safely and comfortably perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants • Ability to use an iPad and clinical workflow for evaluations
