📋 Description

• Provide telephonic patient contact in a remote practice setting • Collaborate with health care professionals to provide meaningful patient experiences • Use critical thinking to develop, implement, and evaluate comprehensive plans of care for patients with multiple disease states • Make outbound calls to educate and support patients regarding new medications • Support continuity of care for defined patient populations through telecommunications nursing outreach programs • Provide customer service to patients using written and verbal communication • Educate patients on medication adherence and disease-state training • Support patients and caregivers with compassion and sensitivity

🎯 Requirements

• Must hold an unrestricted Registered Nurse license in the state of residence • Must have multi-state/compact privileges and ability to be licensed in all non-compact states, territories and the District of Columbia as required by business needs • Must meet applicable state nursing board requirements • Must be based in Texas or Arizona for this requisition • 3+ years of clinical RN experience • Experience using Microsoft Office, including Word, Excel and Outlook • RN Diploma, Associate's or Bachelor's degree in Nursing • Monday-Friday availability, 10:30 a.m.–7:00 p.m. CST set hours • Previous telephonic nursing experience preferred • EPIC systems experience preferred • Bachelor's degree preferred • Licensure in multiple states preferred
