📋 Description

• Examine and treat patients under the direction of the physician. • Conduct outreach to members recently discharged from the ER for follow-up and referral to CareBridge services. • Enroll members into CareBridge services and schedule visits with nurse practitioners. • Triage members, conduct nursing assessments and refer to an acute nurse practitioner. • Review patient medical records, interview patients and measure vital signs. • Explain procedures, perform treatments and administer prescribed medication. • Facilitate patient transfers to alternate levels of care.

🎯 Requirements

• Requires an LPN or LVN and minimum of 2 years of experience as an LPN/LVN; or any combination of education and experience, which would provide an equivalent background. • Current LPN/LVN license in the applicable state required. • Satisfactory completion of a Tuberculosis test required for this position. • Bi-lingual (English/Spanish) preferred. • Previous experience in behavioral health, care coordination, community health, transitional care and/or chronic disease management is strongly preferred. • Must reside within a reasonable commuting distance from one of the posting locations unless accommodation is granted as required by law. • Ability to work four 10-hour shifts weekly, with every other weekend required. • Availability to work 8am-7pm CST.
