📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical histories during Video Visits • Educate members about their health and answer their questions • Help close gaps in care and connect people to appropriate care • Provide 40 hours of availability weekly and maintain 320+ capacity units per month • Maintain a strong visit completion rate with 220+ completed visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Obtain training and credentialing for health plan clients and lines of business in primary coverage states • Participate in regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member-experience KPIs and metrics • Conduct Video Visits remotely while maintaining member privacy and required working conditions

🎯 Requirements

• Full-time availability of 40 hours weekly • Board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • Ability to obtain and maintain multi-state licenses in the coverage area, as required • FNP or AGNP certification preferred for nurse practitioners • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar format preferred • Training and credentialing for all health plan clients and lines of business in primary coverage states • Regular participation in manager, team, and clinical meetings • Minimum remote visit delivery requirements, including broadband connectivity with video and audio enabled • Private, quiet setting with a neutral background • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Ability to sit, write, operate a computer and phone, speak intelligibly, and hear member responses • Ability to view and read a computer screen, iPad, and other electronic devices
