📋 Description

• Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members • Review medications and medical history during Video Visits • Educate members about their health and answer their questions • Help close gaps in care and connect members to appropriate care • Provide Video Visit services only in a private space with adequate internet connection • Participate in efforts to visit millions of people nationwide • Attend regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Operate computers, phones, iPads, and other electronic devices while interacting remotely with members

🎯 Requirements

• Full-time, board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • FNP or AGNP certification preferred for nurse practitioners • Maintain board certification • Able to obtain and maintain multi-state licenses in coverage area, as required • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Maintain training, affiliation, and compliance with Video Visit requirements • Provide 40 hours of availability weekly • Consistently provide 320+ capacity units per month • Consistently pace 220+ completed visits per month • Broadband connectivity with video and audio enabled • Quiet setting with a neutral background for conducting visits • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology
