📋 Description

• Engage patients and healthcare professionals to maximize therapeutic outcomes • Dispense medications and monitor patients’ drug therapies • Work under collaborative practice agreements or approved protocols • Provide consultation and education to patients, caretakers, healthcare professionals, and staff • Gather complete medication histories and reconcile medication lists • Complete comprehensive medication reviews and make evidence-based recommendations • Identify and document suspected adverse drug reactions, medication adherence gaps, and barriers to adherence • Contact providers regarding potentially unsafe or inappropriate medication use and document clinical activities in the electronic record • Serve as a consultation resource on drug therapy and drug products • Assist with discharge planning and help locate medication supplies • Develop, maintain, and manage medication-related guidelines under protocols or collaborative practice agreements • Detect and report medication errors and near misses • Ensure readiness for regulatory and accreditation surveys • Participate in quality improvement and medication use review activities • Prepare and deliver evidence-based best-practice presentations to healthcare professionals • Resolve patient complaints and proactively identify solutions • Communicate between departments and teams to support optimal patient outcomes • Participate in programs and initiatives improving patient care, medication compliance, adherence, and efficiency • Participate in ordering and receiving controlled substances and other medications • Supervise and redirect technicians to maintain an accurate and efficient pharmacy system • Act as pharmacist-in-charge when pharmacy managers and supervisors are absent • Perform registered pharmacist duties in New Hampshire, including approved vaccinations • Render medical assistance when expected as part of the job • Perform other duties as assigned

🎯 Requirements

• Must be currently licensed to practice in the state of NH • Must have a current, unrestricted pharmacist license to practice in the state of NH • Must be willing to seek license reciprocity in multiple states if the position requires it • Doctor of Pharmacy (Pharm D) degree preferred • Completion of a post-graduate residency and/or board certification preferred • Ability to work independently with minimal direct supervision • Ability to set priorities, make critical decisions, respond quickly to emergency requests, handle frequent interruptions, and adapt to changes in workload • Customer-service focus to achieve excellent working relationships • Strong written and verbal communication skills • Ability to perform registered pharmacist duties in the state of New Hampshire, including approved vaccinations • Bloodborne pathogen training is required • Ability to pursue and maintain certifications necessary for system, department, accreditation, and standards compliance
