📋 Description

• Provide input to and lead implementation of DaVita’s transplant roadmap • Increase transplant waitlist rates • Increase deceased-donor and living-donor transplant volume • Develop post-transplant care management capabilities • Collaborate across departments to deploy capabilities across more than 2,800 dialysis centers nationwide and through value-based contracts • Work with physician leaders to increase nephrologist engagement in kidney transplantation • Maintain knowledge of the kidney transplant process, CMS requirements, and UNOS/OPTN regulations • Lead and influence creation of scalable solutions based on data and stakeholder input • Develop and maintain the Village transplant roadmap • Address and implement regulatory and compliance requirements • Prioritize initiatives delivering clinical and business value while improving patient and teammate experience • Influence clinical IT system design to support patient-centered care and reduce teammate and nephrologist burden • Create and support management processes to achieve clinical and operational goals • Analyze data and processes to identify trends and root causes and develop improvement strategies • Ensure communication and training for new resources, BDPs, and model-of-care enhancements • Serve as a transplant thought leader and subject matter expert • Inspire social workers, field leaders, and nephrologists to support patients pursuing transplant • Provide day-to-day leadership to the transplant cross-functional workgroup • Provide regulatory support and navigation • Perform other duties as assigned

🎯 Requirements

• Strong working knowledge of the kidney transplant process • Knowledge of CMS requirements • Knowledge of UNOS and OPTN processes and regulations • Knowledge of key transplant stakeholder needs and roles • Ability to create scalable, data-informed solutions • Ability to address and implement regulatory and compliance requirements • Ability to analyze data and processes to identify trends and root causes • Ability to develop strategies to improve clinical and operational performance • Ability to communicate and train clinical teammates, field operators, and nephrologists • Ability to collaborate across departments and projects • Ability to lead a transplant cross-functional workgroup • Ability to work with physician leaders and nephrologists • Ability to support transplant roadmap implementation across dialysis centers and value-based contracts
