📋 Description

• Supervise the day-to-day operations of the Care Coordinator team in a designated market or geographical region • Organize, monitor, evaluate, and manage workflow • Evaluate roster readiness • Provide work direction and assume responsibility for outcomes • Conduct formal and ad hoc training • Oversee Care Coordinator DKC in-center time and activities • Lead direct reports and the department • Develop and foster a positive, fulfilling work environment • Manage teammate employment activities, including hiring, promotions, performance, and disciplinary actions with appropriate approvals • Address teammate relations issues appropriately and promptly • Facilitate teammate development through PDRs, coaching, mentoring, and training • Collaborate with direct reports to create professional development goals • Review and approve time for non-exempt direct reports • Track overtime, attendance, and PTO • Promote positive relationship management among the team, Neph relationships, and DKC collaboration • Work closely with CSM to support clinical outcomes strategy • Attend and/or lead team meetings, phone conferences, and training as needed • Perform other duties as assigned

🎯 Requirements

• Experience or qualifications sufficient to supervise daily operations of a Care Coordinator team • Ability to organize, monitor, evaluate, and manage workflow • Ability to evaluate roster readiness • Ability to provide work direction and be accountable for outcomes • Ability to conduct formal and ad hoc training • Leadership and people-management capability • Knowledge of teammate guidelines, employment policies, and department/company procedures • Knowledge of basic wage and hour laws and federal, state, and local laws and regulations • Ability to review and approve time for non-exempt direct reports • Ability to track overtime, attendance, and PTO • Ability to work closely with CSM to support clinical outcomes strategy • Ability to attend and/or lead team meetings, phone conferences, and training as needed
