📋 Description

• Reaching out to patients while inpatient to introduce TOC program and communicate post discharge follow up from TOC clinicians • Scheduling post discharge follow ups (PCP/Specialty) • Reaching out to hospitals and provider offices to obtain clinical documentation • Submitting Home Care and DME orders as well as vendor follow up • Monitoring return fax queue and researching provider profile for updated contact and fax information • Provide additional operational support for assigned team and manager as designated • Ensure patients are prepared for telehealth visits by assisting with technology prep • Collaborate internally in clinical rounds to discuss patients at risk for readmission • Coordinate lab testing and imaging, and tracking results to ensure that they are reviewed by the ordering clinician

🎯 Requirements

• 2+ years of patient facing and care coordination experience within healthcare, managed care, home health, and/or community health setting. • Experience working in a telemedicine environment. • Fully bilingual (English/Spanish). • Managed Care / Health Plan experience • MA Certification (desired), expired MA certifications are OK! [Verification: A copy of your active or expired certification will be required. No Certification: If you have significant MA experience/background, we may request copies of your MA school diploma and transcripts.]
