📋 Description

• Deliver high-touch telehealth visits and support the clinical team • Identify care gaps and coordinate member care across a fragmented healthcare system • Communicate and work with PCP offices • Coordinate pharmacy services • Manage durable medical equipment, home health care, imaging, and laboratory testing needs • Prepare for post-visit activities • Manage orders and coordinate care • Coordinate DME and skilled nursing services with home health partners • Coordinate lab testing and imaging and diligently track results • Notify members of lab results and coordinate follow-up care at the clinician's direction • Coordinate medical record collection, information gathering, appointment setup and confirmation, and outbound communications • Direct clinical concerns to the appropriate licensed team • Provide patient education • Advocate for members by answering and returning calls about care coordination issues • Review internal program referrals and send welcome kits and educational materials

🎯 Requirements

• MA Certification highly preferred; expired MA certifications are OK • Verification: a copy of active or expired MA certification will be required • If uncertified, significant MA experience/background may qualify; copies of MA school diploma and transcripts may be requested • Two (2) years of patient facing experience • Telehealth experience is a bonus • Experience in care coordination, particularly helping Medicare patients close care gaps and complete preventative health screenings is desired • Fully bilingual (English/Spanish), and English is desired • Highly organized and adaptable, able to manage a large member panel (100+ if needed) • Strong communication with doctors, pharmacies, nurses, and caregivers • Comfortable working with members via video, phone, and portal • Ability to learn electronic health records and computer platforms including Google Chrome, Google Sheets, AI tools, automated queues, and Advanced Dialer • No clinical decision-making or activities requiring licensure; clinical tasks are performed under licensed professional supervision
