📋 Description

• Lead and supervise a team of TOC Coordinators serving as patient advocates and supporting the TOC clinical team • Ensure patients have resources and support to transition home from hospital and/or post-acute settings and prevent readmissions • Dedicate approximately 30% of time to providing TOC patient care • Manage day-to-day team operations and participate in rounds/huddles • Support the team, develop workflows and job aids, coach staff, and manage performance • Attend and/or facilitate meetings with team members and leadership • Monitor team attendance, adherence, and meeting participation • Collaborate with TOC leadership to develop the program • Represent TOC coordination in cross-functional meetings and share updates • Assist with recruiting and hiring TOC Coordinators • Coordinate pilots involving TOC Coordinators in the TOC clinical model • Partner with the TOC trainer and DM education team to onboard new TOC Coordinators • Manage performance across key performance indicators • Answer questions, monitor relevant Slack channels, and conduct 1:1s • Assist in designing and implementing workflows, creating job aids, and facilitating training • Meet regularly with the Senior Manager to review team performance and recommend improvements • Take on additional responsibilities as the TOC care model evolves

🎯 Requirements

• MA Certification (desired), expired MA certifications are OK! A copy of your active or expired certification will be required. • If you have significant MA experience/background, copies of your MA school diploma and transcripts may be requested. • 2 years of direct lead or supervisory experience in a virtual setting required • 2+ years healthcare administrative/office experience • Proficient in technology using evolving platforms • Organized with high attention to detail • Fully bilingual (English/Spanish) preferred but not required
