📋 Description

• Provide functional and analytical expertise for Epic Healthy Planet implementations, optimization, and ongoing support • Translate value-based care requirements into Epic Healthy Planet workflows, reporting, and analytics solutions • Support population health initiatives focused on quality, cost, utilization, outcomes, and patient engagement • Develop and optimize analytics for value-based contracts, accountable care, and population health programs • Define KPIs and actionable population health metrics with clinical and operational stakeholders • Support reporting and analytics for CMS, HEDIS, MIPS, ACO, Medicare Advantage, Medicaid, and other value-based care programs • Use Epic Healthy Planet data to identify care gaps, high-risk populations, utilization trends, and clinical intervention opportunities • Support risk stratification and segmentation of patient populations • Develop and analyze care gap, registry, quality measure, and population health reports • Partner with clinical teams, care managers, quality teams, and physician leadership to translate analytics into interventions • Analyze performance against value-based contract benchmarks and identify quality and financial improvement opportunities • Support population health and value-based care dashboards and executive reporting • Assist with data validation, reconciliation, testing, and troubleshooting • Collaborate with Epic application teams, reporting and analytics teams, data analysts, and organizational leadership • Participate in requirements gathering, workflow analysis, solution design, testing, training, and go-live support • Evaluate population health processes and recommend workflow, analytics, and operational improvements • Support data-driven strategies to improve patient outcomes and reduce avoidable utilization and cost

🎯 Requirements

• 5+ years of healthcare IT, clinical informatics, population health, or healthcare analytics experience • At least 5 years of demonstrated experience with Epic Healthy Planet project work • Experience supporting Epic Healthy Planet implementations or major optimization initiatives • Strong understanding of value-based care and population health analytics • Experience translating clinical and operational requirements into analytical and reporting solutions • Knowledge of healthcare quality programs and performance measures • Strong analytical, problem-solving, and data interpretation skills • Ability to communicate complex healthcare analytics concepts to technical and non-technical stakeholders • Experience working with physicians, clinical leadership, quality teams, care management, finance, and IT • Strong project management and documentation skills • Epic certification in Healthy Planet required • Preferred: Clarity and/or Caboodle experience • Preferred: SQL and healthcare data analytics experience • Preferred: Tableau, Power BI, or other enterprise analytics platform experience • Preferred: CMS, HEDIS, ACO, MIPS, Medicare Advantage, or Medicaid value-based program experience • Preferred: Risk adjustment, RAF/HCC methodologies, or actuarial analytics experience • Preferred: Payer-provider value-based contract experience • Preferred: Executive dashboard and performance scorecard development experience • Preferred: Experience within large health systems or integrated delivery networks
