📋 Description

• Provide physical therapy treatments according to the established Plan of Care • Work under the direction and supervision of a licensed Physical Therapist • Help patients improve mobility, strength, balance, endurance, and independence • Monitor patient progress and promptly report changes in condition to the supervising Physical Therapist • Review the Plan of Care before each visit • Provide treatment according to physician orders and established clinical standards • Teach patients and caregivers how to safely complete home exercise programs • Educate patients on the safe use of walkers, canes, wheelchairs, prosthetics, orthotics, and other assistive devices • Reinforce patient goals and encourage participation in therapy • Complete accurate and timely clinical documentation • Collaborate with nurses, therapists, physicians, and other members of the care team

🎯 Requirements

• Graduate of an accredited Physical Therapist Assistant program • Current and unrestricted Connecticut Physical Therapist Assistant license • At least six months of experience as a Physical Therapist Assistant preferred • Strong communication, organization, and documentation skills • Ability to work independently in patients’ homes • Willingness to travel throughout Norwalk, Stamford, Greenwich, and the assigned coverage area • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations • Must be able and willing to travel 50% • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
