Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Act as the primary point of contact to maintain communication flow between patients and their care team. • Receive, triage, and resolve incoming clinical messages from patients to clinicians, and relay messages from clinicians to patients as requested. • Conduct regular outgoing patient communications to ensure consistent engagement and optimize program success. • Provide routine administrative support for clinicians. • Contribute to special projects to improve systems and make care more efficient and effective. • Help collect and request medical records from primary care or other providers. • Review patient-uploaded medical records. • Facilitate medical records and prescriptions, including sending lab requisitions. • Assist with prior authorization coordination, medication refills, and pharmacy questions. • Support medical clinicians with pre-visit lab orders. • Guide new patients through the weight loss journey and answer general program questions. • Support account changes or cancellation requests. • Communicate patient feedback to Product, Engineering, Marketing, and other teams. • Assist with clinical data collection for performance indicators and quality improvement. • Provide scheduling and appointment support. • Assist with insurance and billing questions, coverage updates, billing concerns, membership costs, and eligibility questions.

🎯 Requirements

• A Medical Assistant (MA) Certificate is required: Certified Medical Assistant (CMA) from the American Association of Medical Assistants (AAMA) or Registered Medical Assistant (RMA) from the American Medical Technologists (AMT) or Certified Clinical Medical Assistant (CCMA) from the National Healthcareer Association (NHA). • At least two years of experience in a fast-paced primary care, obesity medicine, or endocrinology practice. • Experience with prescription prior authorization processes is a plus. • Excellent customer service skills. • Excellent verbal and written communication skills. • Highly organized with a proven ability to manage multiple tasks and time effectively. • Proficient computer skills, with proficiency in platforms such as Slack and Google Workspace, and the ability to quickly adapt to evolving, custom-built IT systems.
