📋 Description

• Lead end-to-end implementation execution for new practices from kickoff through go-live • Build and maintain implementation project plans, milestones, dependencies, and deliverables • Coordinate AWV workflow implementation, CM embedded rollout, and PAC workgroup activities • Monitor implemented changes, collect outcome data, and identify gaps • Maintain documentation of implementation processes, outcomes, and lessons learned • Pull and compile HubSpot CRM reports for portfolio review, escalation decisions, and leadership reporting • Review CRM and report data for accuracy and completeness before provider-facing delivery • Partner with Data Analytics to resolve discrepancies, gaps, and data integrity issues • Identify practices showing risk indicators and escalate them with supporting context • Manage and track Practice Success tasks, follow-ups, and escalations • Maintain and QA report templates, data sources, and CRM fields used for scorecard delivery • Provide regular status updates and operational insights to leadership • Facilitate internal huddles across CDI, Care Management, and Quality • Collaborate with Clinical Operations, Technology, Quality, and Product teams • Route practice-level operational barriers to pillar leads and track resolution • Coordinate internal operational teams throughout implementation • Incorporate Practice Success feedback into implementation plans, reporting design, and process improvements • Maintain HubSpot CRM records, meeting logs, action items, and engagement status • Update network tracking items for the assigned portfolio • Document meeting notes and distribute action-item summaries • Develop and maintain SOPs, workflow documentation, and process improvements • Identify opportunities to standardize and scale operational processes • Schedule and coordinate internal and cross-functional performance-related meetings • Follow security policies and procedures to protect PHI, PII, and company intellectual property

🎯 Requirements

• Minimum 3 years of experience in healthcare operations, implementation management, project/program management, or healthcare consulting • Strong understanding of healthcare operations and regulatory requirements • Proficiency in data analysis, reporting, and problem-solving • Proficiency with CRM platforms; HubSpot preferred • Proficiency with data/reporting tools • Fluency in the Microsoft suite, especially Excel • Ability to manage multiple projects and implementations simultaneously • Strong written communication skills for internal reporting, meeting notes, action item summaries, and operational documentation • Ability to work independently with strong ownership and accountability • Ability to coordinate and influence across internal cross-functional stakeholders • Experience working with health systems or healthcare provider organizations • Ability to incorporate feedback into improved internal process design • Minimum internet speed of 100 Mbps download and 10 Mbps upload • Employees must be accessible during stated working hours • Ability to join virtual meetings with cameras on • Must follow security policies and procedures protecting PHI, PII, and company intellectual property
