📋 Description

• Develop relationships with patients through telephonic outreach after a clinical event or diagnosis • Collaborate with the clinical team and use health-system resources to improve the patient experience • Research, identify, and examine community resources for patients • Participate in a professional network defining care-guidance standards by disease state • Empower patients to communicate preferences and priorities to healthcare teams and participate in healthcare decisions • Contribute to the development, implementation, and evaluation of the Care Guide navigation program • Address non-clinical factors affecting patient health • Perform other duties supporting company values, mission, and policies • Meet productivity expectations for high-quality, timely client delivery • Maintain regular, reliable attendance during scheduled hours • Support patients and caregivers in resolving practical, social, physical, and emotional barriers

🎯 Requirements

• Proficiency with a personal computer • Technical and analytical skill set • Excellent communication, organizational, and interpersonal skills • Experience with Microsoft Excel preferred • Bachelor's degree preferred; equivalent professional experience will be considered • Bilingual in Spanish and English • Able to work remotely from home in a private HIPAA-compliant workspace • Able to house company equipment needed to perform the job • Broadband internet access • Internet download speed of at least 24 Mbps and upload speed of at least 4 Mbps • Ability to hear in the normal range and wear a headset/earpiece • Good visual acuity for reading computer screens, scripts, and forms • Ability to sit for up to 100% of the time while taking calls • May need to use a personal mobile device for EMR authentication • Must successfully complete a background check, drug screening, and identity verification process • Must be authorized to work without immigration or work visa sponsorship • Regular, reliable attendance during scheduled hours
