📋 Description

• Deliver a seamless, high-touch experience for clients navigating serious and complex medical conditions • Coordinate client engagements, care needs, appointments, services, and follow-up items • Build trusted relationships through responsive, personalized, concierge-level service • Partner with clinicians, researchers, physician offices, and external service providers • Help clients access PHM resources and navigate complex healthcare systems • Proactively support and assess clients’ care needs through direct communication • Initiate client engagements from the welcome email through registration completion • Facilitate scheduling and billing support with physician offices and service providers • Own client meeting details, follow-up, and action-item completion • Support clinical trial eligibility assessment and enrollment processes with researchers

🎯 Requirements

• 1+ year of professional care coordination experience in a complex environment • Ability to provide concierge-level customer service • Exceptional written and verbal communication skills • Unflappable, professional presence • Highest standards of ethics and personal integrity • Demonstrated discretion and care when handling confidential information • Versatility and ability to thrive in a multitasking, fast-paced environment • Positive, energetic, client-focused, can-do orientation • Willingness to bring forward new ideas and perspectives • Strong comfort with technology and ability to quickly learn and navigate new tools and systems • Bachelor’s degree (BA or BS) is nice to have
