📋 Description

• Assess and evaluate members' needs and requirements in a telephonic environment • Guide members and families toward appropriate care and wellbeing resources • Facilitate interaction with resources supporting member care • Employ strategies, approaches, and techniques to manage physical, environmental, and psychosocial health issues • Identify and resolve barriers hindering effective care • Continuously monitor patient care through assessments and evaluations • Ensure patients progress toward desired outcomes • Create member care plans when appropriate • Make independent decisions regarding work methods and courses of action • Work with minimal direction while following established guidelines and procedures • Provide telephonic behavioral health care management • Occasionally work overtime as requested by business needs

🎯 Requirements

• Current valid social service license, e.g. LMSW, LCSW, LPC, LMHC • Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university • 2+ years of experience working as a behavioral health professional • Experience with behavioral change, health promotion, and coaching • Proficiency with Microsoft Office programs, including Word, Excel, and PowerPoint • Experience with shared systems • Ability to troubleshoot and resolve general technical issues • Ability to work an eight-hour shift between 8:00 AM and 6:00 PM in the local time zone • Bilingual English/Spanish proficiency is preferred; fluency in reading, writing, and speaking may be assessed
