📋 Description

• Assess and evaluate members' needs and requirements in a telephonic behavioral health environment • Guide members and families toward appropriate care and wellbeing resources • Facilitate interaction with resources appropriate for members' care • Employ strategies, approaches, and techniques to manage physical, environmental, and psychosocial health issues • Identify and resolve barriers hindering effective care • Continuously monitor patient care through assessments and evaluations • Ensure patients progress toward desired outcomes • Create member care plans when needed • Make independent decisions regarding work methods and courses of action • Work an eight-hour shift between 8:00 AM and 6:00 PM in the local time zone • Occasionally work overtime as requested by business needs • Travel occasionally to Humana offices for training or meetings may be required

🎯 Requirements

• Current valid social service license, e.g. LMSW, LCSW, LPC, LMHC • Master's degree in a behavioral-health field, such as social work, counseling, or related health discipline from an accredited university • 2+ years of experience working as a behavioral health professional • Experience with behavioral change, health promotion, and coaching • Proficiency with Microsoft Office Programs, e.g. Word, Excel & PowerPoint, shared systems, troubleshoot and resolve general technical issues • Experience supporting patients telephonically preferred • Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team, and substance use disorder treatment preferred • Experience working with both children and adults preferred • Bilingual English/Spanish preferred; language proficiency assessment tests reading, writing, and speaking fluency • Download speed of at least 25 Mbps and upload speed of at least 10 Mbps • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
