📋 Description

• Evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Coordinate services and resources using varied support strategies • Identify and resolve barriers to effective care • Monitor patient care through assessments, data, member conversations, and active care planning • Ensure patients progress toward desired outcomes • Facilitate interaction with community health, social service, and other care resources • Travel occasionally to Humana offices for training or meetings

🎯 Requirements

• Bilingual • Bachelor's degree in a health and human services field • 2 or more years of related experience • Ability to use varied electronic information applications and software programs • Passion for continuously improving consumer experiences • Prior experience with Medicare and Medicaid recipients preferred • Experience with electronic case note documentation and multiple computer applications/systems preferred • Experience with health promotion, coaching, and wellness preferred • Knowledge of community health and social service agencies and community resources preferred • Minimum download speed of 25 Mbps and upload speed of 10 Mbps • Wireless, wired cable, or DSL internet connection suggested • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
