📋 Description

• Manage members' health issues using telephonic strategies, approaches and techniques • Identify and resolve barriers that hinder effective care • Monitor patient care through assessments, data, conversations with members and active care planning • Ensure patients progress toward desired outcomes • Guide members and families to essential health and social service resources • Make decisions regarding work approach and priorities while following established guidelines, procedures, regulations and professional standards • Participate in HireVue interviewing as part of the hiring process • Occasionally travel to Humana offices for training or meetings

🎯 Requirements

• Bachelor's degree in a health or social service-related field or LPN licensure and the ability to obtain license in Illinois • 2 or more years of case management experience • Experience utilizing Microsoft Word, Excel and Outlook • Experience with electronic information applications/software programs • Illinois residency preferred • Ability to work Monday–Friday, 8:00am–5:00pm CST, with occasional scheduling flexibility • Internet service with at least 25 Mbps download and 10 Mbps upload speed • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
