📋 Description

• Conduct telephonic assessments to identify members’ clinical, behavioral, social, and environmental needs • Identify and resolve barriers to effective care or treatment adherence • Develop, implement, and update individualized care plans • Monitor member progress toward desired outcomes through ongoing assessment and evaluation • Provide education and support for chronic condition management and self-care • Coordinate care and facilitate access to providers, services, and community resources • Collaborate with members, families, providers, and interdisciplinary teams • Maintain timely and accurate documentation according to guidelines and quality and compliance requirements • Apply department, segment, and organizational policies and procedures • Manage assigned caseloads using independent judgment and available resources • Report to the Associate Director, Health Services Nursing • Travel up to 20% may be required for member outreach and meetings • Primarily work standard business hours, with schedule flexibility based on business needs

🎯 Requirements

• Active, unrestricted Registered Nurse (RN) license in South Carolina with multistate compact privileges and no disciplinary action • Must reside in South Carolina • 3+ years of recent acute care clinical nursing experience • Experience providing education and support for chronic condition management • Experience with community resource coordination • Proficiency with Microsoft Office applications, including Word, Excel, and Outlook • Strong documentation, multitasking, and computer navigation skills with basic troubleshooting • Minimum 25 Mbps download and 10 Mbps upload internet speed • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
