📋 Description

• Evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Coordinate services and resources to support members' optimal wellness • Identify and resolve barriers hindering effective care • Monitor patient care through assessments, data, member conversations and active care planning • Ensure patients progress toward desired outcomes • Make decisions regarding work approach and priorities while following direction and established procedures • Protect member PHI/HIPAA information while working from a dedicated space • Occasionally travel to Humana offices for training or meetings

🎯 Requirements

• Bachelor's degree in health and human services field • 2 or more years of related experience • Knowledge of Microsoft Office Word and Excel • Exceptional verbal/written communication and interpersonal skills • Ability to use a variety of electronic information applications/software programs • Required tuberculosis (TB) screening for this patient-facing role • Valid state driver's license • Personal vehicle liability insurance meeting stated minimum limits or residing-state requirements • Separate room with a locked door usable as a home office • Prior experience with Medicare & Medicaid recipients (preferred) • Experience with electronic case note documentation and multiple computer applications/systems (preferred) • Experience with health promotion, coaching and wellness (preferred) • Knowledge of community health and social service agencies and community resources (preferred) • Home internet with at least 25 Mbps download and 10 Mbps upload
