📋 Description

• Assess members’ health and safety needs and identify strengths, interests, and preferences • Develop comprehensive Member Care Plans (MCPs) • Coordinate with a Field Care Nurse to provide services addressing members’ health and safety needs • Conduct face-to-face social assessments upon enrollment and at least every six months • Conduct quarterly in-person member visits and maintain monthly phone contact • Arrange support services involving social integration, community resources, employment, housing, and other non-medical needs • Ensure cost-effective service delivery • Evaluate risk factors and educate members • Maintain case notes, service authorizations, and MCP updates • Float and cover caseloads for team members in the SW Pod 2 region who have exited their roles or are on leave

🎯 Requirements

• Four-year bachelor’s degree in Human Services or a related field and one year of experience working with at least one Family Care target population, OR a four-year bachelor’s degree in another field and three years of related experience with at least one Family Care target population • Three or more years of Family Care experience • Must reside within the SW region of Wisconsin • Ability to travel up to 40% throughout the SW region • Valid state driver’s license • Proof of personal vehicle liability insurance with at least 100/300/100 limits • Ability to meet Humana’s patient-facing TB screening requirement • Home internet with at least 25 Mbps download and 10 Mbps upload speed • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
