📋 Description

• Integrate programs designed to improve health outcomes, focusing on prescription drugs • Oversee medication therapy and develop strategies for comprehensive medication reviews • Optimize prescription drug use for patients taking multiple medications • Promote health awareness through Rx Education and targeted quarterly campaigns • Make outbound calls to confirm medication adherence and provide counseling • Make decisions about work approach and priorities while following direction and documented procedures • Work independently and contribute to team success • Participate in occasional travel to Humana offices for training or meetings

🎯 Requirements

• Active, unrestricted LPN license • One or more years of experience working as an LPN • Experience with Electronic Health Record (EHR) systems for accurate documentation, data management and workflow optimization • Ability to solve complex problems and encourage collaborative problem-solving across and within teams • Ability to work independently while contributing effectively to team success • Monday through Friday availability • Ability to work assigned shifts between 8:00 AM and 8:00 PM Eastern Time • Ability to work the specified 10:00 AM–6:30 PM ET shift • Ability to work a rotating late shift of 11:30 AM–8:00 PM ET once every four weeks • Ability to meet remote-work requirements: minimum 25 Mbps download and 10 Mbps upload internet service • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
