• Provide clinical care management and population health support for assigned members • Focus on prevention, wellness, and longitudinal health outcomes • Apply nursing judgment to analyze data, identify care needs, and coordinate services using population health management approaches • Conduct clinical assessments to identify member health needs, risks, and care gaps • Develop and implement care plans supporting preventive care, chronic condition management, and wellness goals • Coordinate services across providers, care teams, and community resources • Monitor member progress and adjust care plans based on clinical indicators and outcomes • Provide health education and coaching to support self-management and engagement
• Associate’s degree in Nursing • One (1) year of experience as a Registered Nurse • Active North Carolina Licensure as a Registered Nurse (RN) • Valid driver’s license • Must reside within North Carolina • Must be able to travel within catchment as required • Experience working with BH/MH/SU/IDD population preferred • Knowledge of QM and UM procedures preferred • Experience using data analytics for population health management preferred • CMCN, CCM, ACM, or equivalent care-management or nursing certification preferred • Must pass a drug test as a condition of employment
