📋 Description

• Serve as the primary point of contact for Care Coaches • Maintain team compliance, coaching, and accountability models • Support a team of 15–17 Care Coaches • Produce and support metric-driven reports • Identify barriers and supports to overcome them • Maintain caseload ratios while minimizing mileage and overtime • Monitor patient care through assessments, data, conversations with members, and active care planning • Ensure patients progress toward desired outcomes • Make decisions regarding resources, approaches, and tactical operations for departmental projects and initiatives • Collaborate across departments • Conduct briefings and area meetings • Maintain frequent contact with department managers • Lead regions including Jacksonville, Daytona Beach, and Deland

🎯 Requirements

• Bachelor's Degree in a health-related field • 2+ years of managed care experience in Medicare and/or Medicaid • 1+ years of Quality/Compliance experience • Proficiency in analyzing and interpreting trends • Comprehensive knowledge of Microsoft Word, Excel, and Access • Progressive experience in the health solutions industry, with emphasis on leading and managing teams • Proven track record of leading and motivating teams • Must not exceed 3 hours of driving time to the regions being led: Jacksonville, Daytona Beach, and Deland • Preferred: proficiency in analyzing and interpreting financial trends • Preferred: previous experience in utilization management, discharge planning, and/or home health or rehabilitation • Preferred: experience with health promotion, coaching, and wellness • Preferred: health plan experience • Preferred: 1+ years of management/leadership experience • Minimum download speed of 25 Mbps and upload speed of 10 Mbps for home internet • Work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information
