📋 Description

• Conduct holistic assessments, identify barriers, develop individualized care plans, and adjust interventions to support recovery and self-management across the continuum of care • Coordinate behavioral health services, care transitions, crisis prevention plans, and aftercare with members, providers, and health plan medical management teams • Build trusting, collaborative relationships with members and providers using motivational interviewing, education, and ongoing communication • Serve as the single point of contact for members, facilitating information exchange, discharge planning, and connections to behavioral health, community, and support resources • Ensure accurate and timely documentation and service authorization in accordance with medical necessity criteria, regulatory requirements, accreditation standards, and internal policies • Participate in audits, quality improvement initiatives, case presentations, and ongoing professional development • Support high-quality care delivery, optimal resource utilization, member engagement, continuity of care, and recovery

🎯 Requirements

• Current, unrestricted state license to independently practice as a Clinical Social Worker, Marriage and Family Therapist, Professional Counselor, Clinical Psychologist, or Registered Nurse in the applicable state(s) • 3–5 years of direct clinical care experience in facility-based and/or outpatient behavioral health or substance use treatment settings • Strong clinical judgment and case management skills • Ability to support members through complex care transitions • Excellent verbal and written communication skills, with empathy and clarity • Proficiency in Microsoft Outlook and Word • Ability to document in real time while engaging with members • Ability to interpret and apply medical policy and clinical criteria • Ability to pass background checks upon hire and throughout employment, including criminal, identity, education, employment, sanctions, watchlist, sex offender registry, and drug screening checks • Self-motivated and able to assume responsibility for meeting objectives • Ability to work independently to meet caseload requirements and team objectives • Professional development through specialization, certification, and/or advanced degree is valued • High-speed cable or fiber-optic internet installed before starting, with minimum download speed of 20 Mbps, upload speed of 5 Mbps, and maximum latency of 100 milliseconds • Ability to use computer and phone systems frequently • Ability to remain stationary and seated for extended periods, communicate, inspect information, perform repetitive motions, interpret data, problem solve, make decisions, organize and plan, and maintain a positive professional attitude • Quiet home workspace with minimal background noise • Fixed, designated workspace in the home • Hardwired internet connection and ability to use company-issued wired headsets and desktop equipment
