📋 Description

• Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Manage members' physical, environmental and psychosocial health needs • Manage a caseload and complete assessments in members' homes, community settings and by telephone • Provide clinical support and guidance for members with medical complexity • Develop and coordinate care plans and ensure members receive appropriate services • Address barriers to care and advocate for optimal member outcomes • Review, assess and complete medical complexity attestations and clinical oversight activities • Assess care needs to ensure services are delivered in the least restrictive setting • Develop and modify Individual Care Plans with the interdisciplinary care team • Support members and caregivers in accessing social, housing, educational and other services • Serve as primary point of contact for the Interdisciplinary Care Team • Coordinate with members, care team participants and external resources • Conduct in-home visits 75–90% of the time within assigned Wayne and/or Macomb County areas

🎯 Requirements

• Must reside in the state of Michigan, specifically Wayne or Macomb Counties • Active Michigan license as a Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN), including Nurse Practitioner (NP) or Clinical Nurse Specialist (CNS), with no disciplinary action • 2+ years' experience in health care and/or case management • Ability to travel to homes and community settings for face-to-face assessments • Experience working with the adult population and disease management • Knowledge of community health and social service agencies and additional community resources • Ability to use electronic information applications and software programs, including electronic medical records • Excellent keyboard and web navigation skills • Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel • Valid state driver's license • Personal vehicle liability insurance meeting required limits • Ability to meet home internet requirements of at least 25 Mbps download and 10 Mbps upload • Ability to work from a dedicated space without ongoing interruptions to protect member PHI/HIPAA information • BSN preferred • Experience with in-home assessment and care coordination preferred • Experience with health promotion, coaching and wellness preferred • Experience with Medicaid Long Term Care preferred • Previous managed care experience preferred • Bilingual Spanish, Arabic or Chaldean Neo-Aramaic preferred • Certification in Case Management preferred • Motivational Interviewing Certification and/or knowledge preferred • Required to complete TB screening if selected
