📋 Description

• Perform telephonic and face-to-face assessments and evaluations of LTSS and ABD member needs and requirements • Meet members in the field at their homes or locations of their choice • Assess members' goals, needs and barriers • Connect members and families with appropriate quality services and community resources • Review, analyze, assess and attest for medical complexity • Develop individualized member care plans • Employ evidence-based strategies to manage members' physical, environmental and psychosocial health • Identify and resolve barriers to effective care • Continuously monitor member care through comprehensive assessments and evaluations • Guide members and families toward resources supporting care and well-being • Make autonomous decisions in varied and occasionally ambiguous situations • Work with minimal direction while following established guidelines and procedures • Travel for field interactions with members, families and providers 50–75% of the time • Occasionally attend onsite meetings at the Humana Healthy Horizons office in Glen Allen, Virginia • Support person-centered health outcomes for Humana Healthy Horizons members

🎯 Requirements

• Must reside in the Central Region of the Commonwealth of Virginia in the counties/independent cities of Richmond, Chesterfield, Colonial Heights, Petersburg, Prince George, Hopewell, Charles City or nearby surrounding areas • Active Registered Nurse (RN) license in the Commonwealth of Virginia without disciplinary action • Two (2) years of prior experience in health care and/or case management • One (1) year of experience working with individuals who meet the Cardinal Care Priority Population criteria • Knowledge of community health and social service agencies and additional community resources • Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel • Ability to use electronic information applications/software programs including electronic medical records • Excellent keyboard and web navigation skills • Exceptional communication and interpersonal skills with the ability to quickly build rapport • Ability to work with minimal supervision • Ability to work a full-time schedule • Required to undergo TB screening • Ability to travel to region-based facilities and homes for face-to-face assessments and interactions • Valid state driver's license • Must maintain personal vehicle liability insurance meeting state minimum limits or specified Humana limits, whichever is higher • Dedicated workspace lacking ongoing interruptions to protect member PHI/HIPAA information • Self-provided internet with minimum 25 Mbps download and 10 Mbps upload speed • Monday–Friday availability, 8:00am–5:00pm Eastern Standard Time • Preferred: Bachelor of Science in Nursing (BSN) • Preferred: Certification in Case Management (CCM) • Preferred: Previous managed care experience • Preferred: Motivational Interviewing Certification and/or knowledge • Preferred: 3–5 years of in-home assessment and care coordination experience • Preferred: Experience with disease management, health promotion, coaching and wellness • Preferred: Bilingual or multilingual ability in English/Spanish, Arabic, Vietnamese, Amharic, Urdu or another language; must speak, read and write both languages without limitations and assistance
